1. You authorize Trenchless Supply Inc to inspect and diagnose the equipment described in this request.
2. Diagnostic time may be billable. No repair work above the authorization ceiling stated in this request will be performed without your written approval of a written estimate.
3. Parts and labor are invoiced per the approved estimate. Warranty coverage is subject to the manufacturer's determination.
4. Equipment not picked up within 30 days of the ready-for-pickup notice may incur storage charges.
5. You confirm you are authorized to commit your company to these terms.
